Introduction
Secondary stroke prevention remains a cornerstone of long-term care following ischemic stroke, yet balancing efficacy and safety—particularly the risk of bleeding—continues to challenge clinicians worldwide. Factor XIa inhibitors represent an emerging therapeutic class with the potential to transform antithrombotic strategies by reducing thrombotic risk while minimizing bleeding complications.
